Terms of Service

These Terms of Service govern your access to and use of the medical services, website, and patient portal operated by Integrative & Age Management Medicine, PLLC (“IAM,” “we,” “us,” or “our”), under the medical direction of Dr. William Blackshear. By creating an account or receiving services, you agree to be bound by these Terms.

Medical Services

IAM provides cash-pay concierge medical services including hormone optimization (testosterone, estrogen, and thyroid therapies), peptide therapy (including BPC-157 and PT-141), medical weight management (GLP-1 agonists, semaglutide, and tirzepatide), and sexual health treatments. All clinical decisions are made by Dr. William Blackshear or licensed providers under his supervision.

Eligibility

You must be at least 18 years of age and a resident of the United States to use our services. IAM currently does not serve patients located in Alaska or Hawaii. By using our services, you represent and warrant that you meet these eligibility requirements.

Cash-Pay Services

IAM operates exclusively as a cash-pay practice. We do not bill insurance, Medicare, Medicaid, or any third-party payers. You are solely responsible for the full cost of all services, prescriptions, labs, and supplies received through IAM. We do not submit claims or provide superbills for reimbursement.

Payment & Billing

Payment is due at the time of service or at the time a prescription, lab order, or product is processed. We accept major credit and debit cards. All payments are processed securely through our payment processor. All fees are non-refundable unless otherwise expressly stated in writing by IAM.

Emergency Disclaimer

IAM is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use our website, portal, or messaging system to seek emergency care.

No Guaranteed Outcomes

Medical treatment results vary by individual. IAM makes no warranties or representations that any particular outcome, result, or benefit will be achieved through our services. Treatment plans are personalized, and your results may differ from those of other patients.

Use of AI & Automated Tools

IAM uses artificial intelligence and automated tools to support administrative tasks and clinical decision support, including scheduling assistance, document processing, and preliminary data review. All treatment decisions, prescriptions, and clinical judgments are made exclusively by Dr. William Blackshear or licensed clinical staff. Human oversight is maintained at all times over AI-assisted processes.

Account Responsibilities

You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. You agree to provide accurate, current, and complete information and to update it promptly when it changes. You must not share your account with any other person.

Intellectual Property

All content on our website and patient portal, including text, graphics, logos, and software, is the property of Integrative & Age Management Medicine, PLLC or its licensors and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written consent.

Limitation of Liability

To the fullest extent permitted by law, IAM, Dr. William Blackshear, and their respective affiliates, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services, even if advised of the possibility of such damages. Our total liability shall not exceed the amount you paid for the specific service giving rise to the claim.

Dispute Resolution

Any dispute arising out of or relating to these Terms or your use of our services shall be resolved by binding arbitration in Lake County, Illinois, in accordance with the rules of the American Arbitration Association. You waive any right to a jury trial or to participate in a class action. Illinois law governs these Terms.

Termination

IAM reserves the right to suspend or terminate your account and access to services at any time, with or without notice, for conduct that we determine violates these Terms, is harmful to other patients or staff, or is otherwise inappropriate. You may also discontinue use of our services at any time.

Changes to Terms

We may update these Terms from time to time. We will notify you of material changes by posting the updated Terms on our website or by sending a notice to the email address associated with your account. Your continued use of our services after the effective date of any changes constitutes your acceptance of the revised Terms.
